چکیده

The free vascularized fibular graft (FVFG) to the tibia was first described by Taylor et al in 1975.1 Since then, the operation has evolved to include transfer of adjacentmuscle, and in 1983 Chen andYandescribedanosteocutaneousflap.2These advancements have provided a limb-sparing option to those patients once destined for amputation. Indications for these procedures now include trauma, chronic osteomyelitis, wide resection of malignant tumor, and pseudoarthrosis of long bones.3 Prior to the advent of these procedures, complex fractures of the tibia were treated with autologous, nonvascularized cancellous bone grafting. Nonunion and complication rates were high, often leading to delayed amputation. Nonvascularized fibular autografts also showed high complication rates and particularly high rates of nonunion and stress fractures. Attempts to reduce these complications led to the proposal for vascularized grafts. Free vascularized flaps have quicker

Long-term effect on foot and ankle donor site following vascularized fibular graft resection in children.

This study was carried out to evaluate the long-term effect on the donor side of the foot and ankle following vascularized fibular graft resection in children. Eight patients underwent resection of the fibula for the purpose of a vascularized fibular graft by a surgical team who practiced leaving at least 6 cm residual distal fibula. The age of these children at the time of surgery was between ...

Reconstruction with double pedicel fibular graft and ankle arthrodesis for aggressive chondroblastoma in the distal tibia

BACKGROUND Aggressive chondroblastoma of the distal tibia is rare, and below-knee amputation had been the standard surgical procedure. CASE PRESENTATION We reported an additional case and reviewed the existing literature. A 20-year-old man with a 2-month history of right ankle pain and swelling underwent distal tibia wide resection, double pedicle fibular, autogenous iliac bone graft, and ank...

MICROSURGICAL TREATMENT OF LONG BONE DEFECTS USING FREE VASCULARIZED FIBULAR TRANSFER

Management of long bone defects occurring in accidents or war injuries is a difficult problem, From 1978 to 1986, 11 patients were treated for long bone defects of the humerus ( 10 cases) and radius (one case), using free vascularized fibular transfer by microsurgical technique .The patients were followed for 4 months and in all of them, selective angiography and scanning revealed exellent...

Primary Ankle Arthrodesis for Severely Comminuted Tibial Pilon Fractures.

Management of severely comminuted, complete articular tibial pilon fractures (Rüedi and Allgöwer type III) remains a challenge, with few treatment options providing good clinical outcomes. Twenty patients with severely comminuted tibial pilon fractures underwent primary ankle arthrodesis with a retrograde calcaneal nail and autogenous fibular bone graft. The fusion rate was 100% and the varus m...

A free vascularized tibia-fibular composite graft for the traumatic femoral bony defect of a 6-year-old boy with 10-year follow up: a case report

INTRODUCTION Free vascularized fibular grafts have been widely used for the reconstruction of long bone defects. However, the use of a vascularized tibial graft is precluded by its weight-bearing function and unacceptable donor site morbidity. CASE PRESENTATION We present a rare case of using a vascularized tibia-fibular composite graft taken from a 6-year-old Chinese boy's ipsilateral lower ...
